1. Click the form number or name to download the form to your computer. At this point, be sure to save the PDF file to your desktop or documents folder.
2. Nearly all of our forms can be filled out on your computer using Acrobat® Reader. You can also print the form and fill it out manually if you'd like.
3. When filling out surety bond applications it is VERY IMPORTANT that the application is accurate and complete with current information. This will avoid the inevitable delays in obtaining your surety bond.
4. Before sending the application back to us, verify it has been signed by all parties that apply. If you have questions, or need any assistance in filling out the application please call us.
5. If filling out the application by hand please print very clearly! Some of the information on the application will end up on the surety bond. If it's incorrect, the surety bond will be rejected.
6. If you use Acrobat® Reader to fill out the application be sure to save, print and sign the application when you are finished.
7. Mail or fax the completed and signed application to Mountain High Surety. Be sure to keep a copy for your own records. You can find our mailing address and fax number here.
